I like the addition of a Tag Team title (maybe call it the Tag Team Extreme Champions or something)...and perhaps a universal (i.e., ANYONE can go after this title...though it will change hands probably a TON more than the others) Heart Attack Championship.

Ok...those title names are OBVIOUSLY not etched in stone...but that's my idea.
